脊髄損傷

症状

脊髄損傷は、損傷レベル以下の運動・知覚麻痺とともに自律神経の障害、そして脊髄ショックなどが複雑に絡み、前述した症状以外にも、様々な症状をきたします。

呼吸障害

損傷レベルによっては横隔膜や肋間筋などの呼吸筋が障害されてしまう。

肋間筋が傷害されると、呼吸による肺の収縮に肋骨が追従しなくなるため、呼吸がしづらくなります！

排尿障害

脊髄ショック時に尿閉、排尿困難となる。

尿が逆流することがあり、それが原因で腎盂腎炎となってしまい、命の危機に瀕する場合があります！！

循環器障害

感覚の消失や排尿障害などによる褥瘡や膀胱の貯まりすぎ、あるいは便秘などにより自律神経が圧迫され、自律神経過反射の引き起こす。

これにより高血圧が起こることがある。

Th5-6以上の脊髄損傷でみられる合併症。

症状
鳥肌, 発汗, 顔面紅潮, 徐脈, 血管攣縮, 高血圧

原因
膀胱の充満, カテーテルの詰まり, 腸の過膨張, 皮膚の感染症, 褥瘡などが刺激となり発生する。

対処
臥位の場合は、頭部を挙上し血圧を下げる。
原因を見つけ、取り除く。

このような場合の血圧を下げるために、車椅子にはティルト機能やリクライニング機能が求められます。

消化管障害

まれに脊髄ショック時に腸管の蠕動運動が麻痺を起こすことがあり、排便障害が持続する。

特に頸髄や上位胸髄損傷で起こりやすい。

治療のためにステロイドを注入した際に、上部消化管出血の危険性が高まる。

体温調節障害

発汗機能などの自律神経機能が障害されるため、体温調節機能に異常をきたす。

過高熱、低体温などがみられる。

“できるだけ暑いところにいないようにする”、“外出した際には首や脇を中心に冷やす”などの対策が必要になります。
