神経伝導路！〜脳幹から脊髄〜

神経伝導路

神経伝導路とは脳や脊髄の内にある神経（ニューロン）の通り道のことです。

特に運動ニューロンに関わる神経伝導路は通過するポイントやどこに重きを置くかによって、様々な名称が存在します。

具体的な例を挙げながら紹介していきます！

スタート地点とゴール地点による名称

筋でいうところの起始部と停止部のように、ニューロンの起始部と停止部の名称から名付けられるタイプです。

例：皮質脊髄路

皮質脊髄路の場合、“大脳皮質と脊髄をつないでいる運動ニューロンの通り道”になります。

名古屋市（愛知県）と神戸市（兵庫県）を繋ぐ“名神高速道路”のようなネーミングイメージです。

形状の特徴による名称

特徴的な形状をもつことから、その箇所の名称がつけられているタイプです。

例：錐体路

後述する脳幹の“延髄”で、腹側面の正中線の両側で“錐体”という線維束の膨らみをつくるところから“錐体路”と呼ばれています。

錐体路

名称は違うが、皮質脊髄路と同じ伝導路。

後述する“延髄”にて詳しく紹介しますが、“錐体交叉”と呼ばれる特徴的な交通の仕方をしています！

これは脳血管障害における麻痺症状の出方に大きく関わっています！！

錐体外路

錐体路と錐体外路    錐体外路系のループ ※ 拡大可能

  • 錐体外路は、延髄の錐体路以外の運動路を意味する
  • 錐体外路は“制御系皮質核路”により抑制的に制御されており、制御系皮質核路は大脳基底核と小脳により調節されている
  • 全体を“錐体外路系”と呼び、脊髄前角細胞の興奮を調節している

運動において錐体外路は筋緊張や姿勢といった意識して行わない運動のコントロールに関与している。

錐体外路系に障害が生じると、代表的な疾患としてパーキンソン病を発症する可能性がある。

ニューロンの伝導方向による名称

神経伝導路内でのニューロンの伸び方、あるいは内部司令や外的刺激の伝導する方向によって名称がつけられているタイプです。

運動・感覚ニューロンのつながり ※ 拡大可能

上行性伝導路（求心性神経線維）

  • 末端から中枢へと伝導する神経伝導路
  • 感覚ニューロンがそれにあたる

下行性伝導路（遠心性神経線維）

  • 中枢から末端へと伝導する神経伝導路
  • 運動ニューロンがそれにあたる

神経核を基準とした名称（神経伝導路ではない）

運動ニューロンは脊髄の前核と呼ばれる箇所に核をもっていて、その核より上（近位）か下（遠位）かで名称がつけられているタイプです。

この名称は麻痺の分類で使われることが多いです（核上性麻痺など）。

核上性線維

  • 脊髄前角細胞より近位の神経線維
  • “上位運動ニューロン”とも呼ばれる

核下性線維

  • 脊髄前角細胞より遠位の神経線維
  • “下位運動ニューロン”とも呼ばれる

名称は違うけど実は同じ伝導路
